Bay Medical Group are looking to recruit a number of Pharmacists for a fixed 10-month period to cover maternity leave. Full and part-time hours are available. Main duties of the job
- Collaborating with a team of pharmacists, technicians, and other members of the Medicines Management Team (MMT).
- Working as an integral part of a multi-disciplinary team, contributing both administratively and in direct patient-facing interactions.
- Conducting clinical medication reviews to proactively manage patients with complex polypharmacy, ensuring their treatment regimens are both safe and effective.
- Serve as a primary point of contact for general practice staff, providing expert advice on prescription and medication queries. They will play a crucial role in supporting the repeat prescription system.
- Offer expert clinical guidance on medicines, addressing the public and social care needs of patients.
- Take a leading role in medicines optimisation and quality improvement, overseeing aspects of the quality and outcomes framework as well as enhanced services.
- Ensuring seamless integration between community and hospital pharmacies.
- Contribute to the efficient use of skill mix, enhancing patient outcomes.
- Improving Healthcare access and helping to manage workload.
- This role is vital in driving improvements in both the quality of care and operational efficiencies, requiring a high level of motivation and passion for delivering exceptional service within general practice.
Bay Medical Group is proud to be the general practice serving the population of Morecambe and Heysham. We operate out of 5 sites and deliver general medical services to c54,000 patients.
Formed in 2016/17 by established legacy practices in the town, we are fortunate to be a single at scale provider in a distinct geographical area and as such were able to establish the Primary Care Network (PCN) for Morecambe and Heysham in July 2019, with Bay Medical Group as the only Practice in the PCN.
