HOW TO REGISTER FOR PATIENT ACCESS
Patient Access is a great online service which allows you to book appointments, order repeat prescriptions and view your medical records. The services is open 24/7/365 and can be accessed from your home PC, Tablet or Mobile phone. Please note, medical record viewing is for those aged 16 or over only.
Register at the surgery in person
Step 1. download, complete and print our form
Step 3 Visit the Practice in person and hand to reception your completed application form.
Please Note that we also have the application form at reception if you are unable to download, complete and print it.
Step 4 Show your Identification if over 16 - two forms of ID. one photo ID such as Passport, drivers licence, freedom pass etc and also one form of ID with your home address such as recent utility bill, bank statement etc. Full List of acceptable ID
Once you have completed the above steps, the Practice will process your application and send you your registration details by post.
Simply follow the instuctions given to you in the letter.
Once you have registered your Patient Access account, you will have full access to all the online services you requested. Please remember to take note of your User ID as this is needed to log in each time.
Register over the phone
Call the surgery on 01524 511999 and advise the Patient Advisor you wish to register for online services and they will ask you a few questions realting to your healthcare, to confirm your identity. They will then task a team who configure this for you and contact you with your Linkage Key (this may take a few days depending on workload).
