At Bay Medical Group we continually look for ways in which we can improve the care we provide and as part of the Integrated Care Community we are excited to be able to offer the post of Clinical Care Coordinator.

We are looking for someone who can bring their knowledge and experience of working within the health and social care sector, who takes a personalised care approach to work holistically with patients. The job is suitable for someone currently registered as a clinical professional e.g. Nurse, Physiotherapist, Occupational Therapist, Paramedic and is an opportunity for the right candidate to use their skills, knowledge and experience to work across sectors to support improved outcomes for our patients.

Working closely with the Clinical Leads, Social Prescriber, Care Coordinator, and various primary, secondary, and third sector stakeholders within the wider Bay Integrated Care Community, you will join our friendly team delivering care and support to vulnerable patients.
