District Nursing Team and Nurse Visitors
The district nurses provide nursing care and support to patients in their own homes when they are too ill to attend surgery. The team provides specialist nursing care as necessary. This care includes wound care, leg ulcer management, palliative care, advice and support in maintaining health and referrals to appropriate heath professionals/social services as required. The Nurse Visitors for Older Adults can assess, implement and advise on all aspects of care and services that are available for the housebound older adults and those with long term conditions.

The health visitor can advise on health care to all age groups but is mainly involved with mothers, babies and young children. Their work includes child health surveillance, childhood immunisations & general health promotion.

Provide antenatal care throughout pregnancy, during labour and after birth. If newly pregnant, you can now self-refer for an appointment with a Midwife.
Medicines Management Team
Process and deal with all prescription requests that come into the surgery. The MMT work closely with the local pharmacies nursing and care homes.
Based at Heysham Primary Care Centre
Our Medicines Management Team, in addition to having a team ensuring prescriptions are issued in a timely manner, provides a comprehensive service for our patients. We have a team of experienced pharmacists, including pharmacist prescribers, available to ensure the needs of our patients are met.They advise patients on all aspects of their medicines, ensuring these are safe and appropriate. They liaise with secondary care about any medication changes suggested from there. They are available to answer specific queries from clinicians within the practice to ensure our patients receive the most suitable treatments. Our pharmacists deal with a wide variety of urgent queries from patients, working closely with the multidisciplinary team within the practice.
