ABOUT PATIENT ACCESS
Having online services like Patient Access allows you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:
- book or cancel appointments online Please note: due to the coronavirus pandemic, not all appointments are avilable to book online as calls must be triaged for patient safety.
- renew or order repeat prescriptions online
- view your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results
Being able to book appointments online, will free up the phone lines for patients who don`t have access to the internet or have a smartphone
Being able to order your own prescriptions and have it sent electronically to the pharmacy of your choice is a real time saver
Being able to see your record online might help you to manage your medical conditions. It also means that you can access it from anywhere in the world should you require medical treatment on holiday. Please note, medical record viewing is for those aged 16 or over only.
The services is open 24/7/365 and can be accessed from any PC, Tablet or Mobile phone.
HOW TO REGISTER FOR PATIENT ACCESS
Register at the surgery in person
Step 1: Download and print our form
Under 16's or carers of an under 16 must complete this form - Online Services Application Form (under 16's) This is for appointment booking and prescription ordering ONLY
Over 16's registering for themselves do not require a form
If you are a carer of an over 16 you must complete this form - OnlineServices Application Form (over 16's) for Carers
Step 2 : Visit the Practice in person and hand to reception your completed application form.
Please Note that we also have the application form at reception if you are unable to download, complete and print it.
Step 3 : You will be required to prove your identity
Once you have completed the above steps, the Practice will process your application and send you your registration details.
Register over the phone
Call the surgery on 01524 511999 and advise the Patient Advisor you wish to register for online services and they will ask you a few questions relating to your healthcare, which is to confirm your identity. They will then task a team who will configure this for you and contact you with your details (this may take a few days depending on workload).
