Introduction

To help us synchronise your medication, please complete the following form.

Please be aware that we cannot synchronise certain medications for example patches, creams, any liquid medication, inhalers or injections.

When you next collect your medicines you will receive different quantities of each item to bring them in line.

In the future, you might need to synchronise your medications again, as they may come in different pack sizes for example 28 or 30 tablets per pack.
