From the 1st of April 2025, Bay Medical Group will be using a new system called Medlink for reviewing long-term conditions (LTCs) online. The system is being used in many GP Practices across the country with success. It also provides patients with links to useful resources to support your health which you should find very useful.

Aim

The new system will assist in gathering the data necessary for reviewing all your relevant condition(s) in the easiest way possible, reducing the number of appointments in the surgery you require.

In some cases, you may not require a face-to-face review if the clinician is happy with the information, you have provided and how your condition is currently being managed. However, if you would like a follow up, you can state this in your questionnaire.

How does it work?

You will be sent an invitation by email or text message, if you use the NHS App, your message will arrive here first, this has a link to complete your personalised health questionnaire. Invitations will be sent around your birthday month. If you do not have access to email, the NHS App or text messages, you will receive an automated telephone call asking you to contact the surgery to book in for your review.

When you receive your email or text please click on the link and complete your online review. This can be completed on a smartphone, tablet or computer; you can copy or type the link into a desktop computer browser to use a larger screen.

Depending on the condition(s) you have, you may be asked to book an appointment for example a blood test, or a foot check in the surgery to complete the data collection part of your review.

Once all the results are back, they are sent to an appropriate clinician to review. They will let you know whether everything is satisfactory and if your review is complete, or whether a follow up is needed.

I can’t use the internet, what should I do?

If you can’t complete the health questionnaire online you can call us on 01524 235900 option 1 to book your review.

Once you have clicked the link you will be given more information about the online review you may need to compete (screenshot below).

Your responses are saved automatically as you go, so if you can’t complete it in one sitting, you can return at any stage by re-clicking on the link again in your email or text and continue later. Feel free to ask a family member or friend to help you answer the questions.

After completing your review online, you may receive instructions if a physical check appointment (for example, a blood test or blood pressure) is needed.

When we have all the information needed to assess your conditions, a clinician will then review your online responses and physical check results and decide if further interaction or discussion with you is required. We will email or text you to let you know next steps, if we would like to speak with you on the phone or we would like to book an appointment to see you face to face we will send you the information you need to book the relevant appointment on the NHS app. Or we may decide that we are happy with your control and will be in touch in another year.
