Pharmacist

Bay Medical Group are looking to recruit a Pharmacist. 

Main duties of the job:

• Serve as a primary point of contact for general practice staff, providing expert advice on prescription and medication queries. 
• Work as an integral part of a multi-disciplinary team, contributing both administratively and in direct patient facing interactions.
• Conduct clinical medication reviews to proactively manage patients with complex polypharmacy, ensuring their treatment regimens are both safe and effective.
• Offer expert clinical guidance on medicines, addressing the public and social care needs of patients.
• Ensure seamless integration between community and hospital pharmacies.
• Drive improvements in both quality of care and operational efficiencies, requiring a high level of motivation and a passion for delivering exceptional service within general practice.
