District Nursing Team and Nurse Visitors

The district nurses provide nursing care and support to patients in their own homes when they are too ill to attend surgery. The team provides specialist nursing care as necessary. This care includes wound care, leg ulcer management, palliative care, advice and support in maintaining health and referrals to appropriate heath professionals/social services as required. The Nurse Visitors for Older Adults can assess, implement and advise on all aspects of care and services that are available for the housebound older adults and those with long term conditions.

The health visitor can advise on health care to all age groups but is mainly involved with mothers, babies and young children. Their work includes child health surveillance, childhood immunisations & general health promotion.

 

Community Midwives

Available at all sites

Provide antenatal care throughout pregnancy, during labour and after birth. If newly pregnant, you can now self-refer for an appointment with a Midwife
